The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside GPs, nurses and the admin team in general practice. In this role they will be supported by a GP partner who will develop, manage and mentor them and be part of a network wide pharmacist forum. The post holder will work as part of a multi-disciplinary team in a patient-facing role dealing with patient facing medication queries. The postholder will take responsibility for areas of long term chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

We are seeking an enthusiastic and motivated Clinical Pharmacist to join our multidisciplinary primary care team at The Hollies Medical Centre. The successful candidate will work alongside GPs, nurses and administrative staff to deliver high-quality patient‑centred care, with a focus on medicines optimisation, chronic disease management and safe prescribing. Key responsibilities include undertaking clinical medication reviews, managing complex polypharmacy, supporting repeat prescribing systems, responding to medication queries, medicines reconciliation following hospital discharge, and supporting care home medication reviews. The role also involves contributing to prescribing audits, quality improvement initiatives, QOF and enhanced services, and promoting safe and cost‑effective prescribing in line with NICE and local guidance.

Experience

  • Experience of working as a pharmacist in a patient‑facing role.
  • Knowledge and understanding of acute and long‑term conditions commonly managed in general practice.
  • Experience of conducting clinical medication reviews and managing polypharmacy.
  • Experience of dealing with medication queries and supporting safe prescribing.
  • Ability to work effectively within a multidisciplinary healthcare team.
  • Experience of using clinical systems and maintaining accurate patient records.
  • Understanding of NICE guidance, prescribing protocols and medicines safety alerts.
  • Previous experience working in general practice or primary care.
  • Experience of medicines reconciliation following hospital discharge.
  • Experience supporting care homes and vulnerable patient groups.
  • Involvement in prescribing audits, quality improvement or QOF activities.
  • Experience supervising or mentoring pharmacy staff or trainees.

Qualifications

  • Masters Degree in Pharmacy (MPharm) or equivalent.
  • Registered Pharmacist with the General Pharmaceutical Council (GPhC).
  • Evidence of continuing professional development (CPD).
  • Understanding of medicines optimisation and prescribing governance.
  • Postgraduate qualification in clinical pharmacy or primary care.
  • Training or experience in long‑term condition management.
  • Experience of working within Primary Care Networks (PCNs).
