We are seeking an enthusiastic, experienced, and highly motivated Full or Part Time Practice Nurse to join our supportive nursing team on a fixed-term basis to cover maternity leave. The role is ideal for someone who is passionate about patient care and proactive in improving quality within a Primary Care setting.

Main duties of the job

  • Hold more than one academic Level 6 diploma or postgraduate certification in long-term conditions care and/or public health initiatives.
  • Demonstrable experience in leading long-term conditions management and quality improvement.
  • Is registered with the NMC and maintains revalidation in line with NMC requirements; and is working at Registered Nurse Level Practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework.
  • Provide and maintain our high standards of nursing care.
  • Duties will include all tasks normally undertaken by an experienced Practice Nurse and any additional roles agreed between the nurse and the doctors as appropriate, having regard to current training.

Job responsibilities

Key Responsibilities

  • Providing high-quality, evidence-based nursing care in line with clinical protocols.
  • Assessment, planning, implementation, and evaluation of care for patients with, or at risk of, long-term conditions.
  • Leading quality improvement initiatives and managing patients at risk of developing chronic illnesses.
  • Administering childhood and adult immunisations, travel vaccinations, and cervical screening.
  • Delivering health promotion and lifestyle advice on areas such as smoking cessation, diet, and physical activity.
  • Performing wound care, suture removal, and management of minor illnesses within scope.
  • Providing contraceptive services, including Depo-Provera injections, and general sexual health advice.
  • Supporting infection prevention and control (IPC) activities including audits and training.
  • Maintaining accurate and confidential clinical records in line with legislation and best practice.
  • Attending relevant training sessions and keeping up to date with CPD and professional development.

Experience

  • Previous experience working autonomously.
  • Experience as a Practice Nurse or Community Nurse.
  • Working with Learning Disability, elderly or housebound patients.
  • Involvement in quality improvement initiatives.
  • Willingness to undertake further training and development.

Qualifications

  • NMC Registered Nurse (with active registration and compliance with revalidation).
  • More than one Level 6 diploma or postgraduate qualification in long-term condition care and/or public health initiatives.
