This post is designed to provide clinical experience in Acute Medicine with a focus on Same Day Emergency Care (SDEC). The Junior Clinical Fellow will contribute to the safe and efficient assessment, investigation, and management of acutely unwell patients suitable for ambulatory pathways, working as part of a multidisciplinary team to improve patient flow and reduce unnecessary admissions. The role also offers opportunities for professional development, teaching, quality improvement, and research in line with the Trustβs strategic priorities.
About SDEC
Our Same Day Emergency Care (SDEC) service provides rapid assessment, investigation, and treatment for patients with a wide range of acute medical conditions, including:
- Infections
- Pulmonary embolism
- Deep vein thrombosis
- Chest pain
- Other ambulatory acute medical presentations
The unit is staffed by a dedicated SDEC Consultant, supported by a Physician Associate (PA), Advanced Nurse Practitioner (ANP), and an experienced nursing and administrative team. Medical SDEC plays a key role in supporting the Emergency Department, Urgent Treatment Centre (UTC), and acute inpatient services, helping reduce unnecessary admissions and improve patient flow across the Trust. The service operates 7 days a week (08:00β20:00 weekdays, 08:00β16:00 weekends). This role will not involve overnight or weekend on-calls. Shift patterns range from 08:00 β 16:00 and 10:00 β 18:00.
Responsibilities
- Provide timely assessment, investigation, and management of patients referred to SDEC
- Review patients and formulate management plans under consultant supervision
- Participate in ambulatory pathways including VTE, chest pain, infection, and frailty where relevant
- Ensure timely documentation and safe handover of care
- Work collaboratively with ED, UTC, community teams and ward colleagues to support early supported discharge and admission avoidance
- Participate in SDEC safety huddles, board rounds and MDT discussions
Professional & Educational
- Attend clinical and educational meetings
- Undertake supervised learning and skills development relevant to urgent and ambulatory care
- Participate in simulation training, acute deterioration training and other in-house programmes
- Participate in teaching junior trainees, medical students, and other learners
Quality & Governance
- Participate in audit, service evaluation and quality improvement projects
- Contribute to patient safety processes, incident reporting, and reflection activities
- Support development of pathways and innovation within SDEC
General Responsibilities
- Maintain accurate, timely and legible clinical records
- Demonstrate adherence to GMC Good Medical Practice
- Participate in appraisal and PDP planning
