As our service continues to grow, we are looking for an experienced, organised and proactive Care Coordinator to join our friendly and supportive team at our Sutton office.

The Role

As a Care Coordinator, you will play a vital role in ensuring our care services are delivered safely, efficiently and to the highest possible standards.

You will be responsible for coordinating care packages, managing staff rotas, liaating with clients and healthcare professionals, and ensuring continuity of care.

You will work closely with the Registered Manager, Deputy Manager and Field Care Supervisors to ensure every service user receives safe, responsive and person-centred care.

Key Responsibilities

  • Coordinate and schedule care staff efficiently using electronic rostering systems.
  • Allocate new care packages whilst maintaining continuity of carers wherever possible.
  • Manage staff rotas, sickness, annual leave and emergency cover.
  • Liaise with Local Authorities, NHS teams, hospitals, social workers and other professionals regarding referrals and care packages.
  • Build positive relationships with service users and their families, ensuring concerns are resolved promptly.
  • Support with the onboarding of new care packages, ensuring all required documentation is completed.
  • Assist in reviewing and updating care plans, risk assessments and support documentation.
  • Monitor call monitoring systems to ensure visits are completed on time.
  • Respond effectively to changes in client needs and arrange appropriate care.
  • Carry out quality assurance checks and contribute towards service improvements.
  • Participate in the on-call rota where required.
  • Ensure compliance with CQC Fundamental Standards, company policies and safeguarding procedures.
  • Support recruitment by assisting with interviews, inductions and staff allocation.
  • Maintain accurate records using digital care management systems.
  • Work collaboratively with the wider management team to achieve service targets and excellent outcomes.

What We're Looking For

We're looking for someone who is passionate about delivering exceptional care and enjoys solving problems in a busy office environment.

Essential

  • Previous experience working as a Care Coordinator within a domiciliary care or home care setting.
  • Excellent knowledge of care scheduling and rota management.
  • Strong understanding of CQC regulations and domiciliary care compliance.
  • Excellent communication and interpersonal skills.
  • Ability to remain calm under pressure and prioritise competing demands.
  • Strong organisational and time management skills.
  • Good IT skills, including Microsoft Office and electronic care management systems.
  • Excellent attention to detail.
  • Ability to build positive relationships with staff, clients and professionals.
  • Full UK driving licence and access to your own car

Desirable

  • NVQ Level 3 (or above) in Health & Social Care.
  • Experience using care management software.
  • Experience working with Local Authority and NHS commissioned services.
  • Knowledge of safeguarding, Mental Capacity Act and person-centred care planning.
