Groves Health is looking for an experienced and motivated Practice Nurse to deliver high quality care to patients at our New Malden Surgery. Ideally, we are looking for a Qualified General Practice Nurse to work 4 days a week. Working hours will be by arrangement. You may be required to work at one of our other sites on occasions, which are in: Richmond, Wimbledon and New Malden.
Main duties of the job:
- Conduct thorough assessments of patients' health conditions.
- Formulate and implement care plans.
- Provide treatment as necessary.
- Perform treatment room duties and CDM clinics.
- Collaborate with other healthcare professionals to ensure optimal patient care.
- Maintain accurate and up-to-date patient records.
- Offer health education and counselling to patients and their families.
- Stay current with the latest medical advancements and best practices.
Job responsibilities:
- Identify, manage, and support patients at risk of developing long-term conditions.
- Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
- Provide vaccination and travel medicine services.
- Provide wound care (ulcer/ Doppler etc.) to patients.
- Request pathology services as necessary.
- Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/ specialist care as required.
- Maintain accurate clinical records in conjunction with extant legislation.
- Ensure snomed codes are used effectively.
- Maintain chronic disease registers.
- Maintain vaccination programmes for adults and children.
- Provide cervical cytology.
- Prioritise health issues and intervene appropriately.
- Recognise, assess, and refer patients presenting with mental health needs.
- Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice).
- Support the clinical team with all safeguarding matters, in accordance with local and national policies.
- Deliver opportunistic health promotion where appropriate.
- Monitor and maintain baby immunisation targets.
- Monitor cervical smear results.
- Manage emergency drug kit.
- Attend clinical and practice meetings and clinical supervision.
Person Specification:
Experience:
- Essential: 2 years' relevant experience working in a primary care environment in the last 2 years.
- Desirable: Experience of chronic disease management.
Qualifications:
- Essential: Nursing degree or equivalent experience, Registered Nurse (NMC), Evidence of immunisation training (childhood, adult and travel health), Experienced in complex wound dressings/leg ulcer management, Evidence of cervical cytology training.
- Desirable: Standards for Students Supervision & Assessment (SSSA), Diabetes Diploma, Family Planning qualification.
Clinical Knowledge & Skills:
- Essential: Immunisations (flu, routine, childhood, B12, travel), Wound Care/ Treatment of small injuries/ Removal of sutures & staples, Women's health (Cervical cytology, contraception, etc.), Dressings, Diabetes review, Hypertension, Cardiovascular disease - primary prevention, Coronary Heart Disease, Ability to record accurate clinical notes, Ability to work within own scope of practice and understanding when to refer to GPs, Understanding of extended brief interventions (NICE guidelines), Cytology certificates, Travel medicine, ECGs, Venepuncture, Asthma review, Understands the requirement for PGDs and associated policy, Chaperone procedure.
- Desirable: Knowledge of Mental Health conditions, Requesting pathology tests and processing the results, advising patients accordingly, Understands the importance of evidence-based practice, Knowledge of public health issues in the local area, Awareness of issues within the wider health arena, Knowledge of health promotion strategies.
