Are you passionate about helping people make lasting change? We’re looking for a Senior Health & Wellbeing Practitioner to join our Integrated Substance Misuse Team at HMP Usk & HMP Prescoed, working full time (37.5 hours per week). This is a rewarding leadership role supporting adult males within a prison setting, combining frontline clinical work with team management and service development.

You’ll play a key role in supporting individuals with substance misuse and low-level mental health or emotional wellbeing needs, helping them progress through structured recovery journeys and achieve positive outcomes.

  • Line managing Health & Wellbeing Practitioners and supporting their development
  • Carrying out assessments, care planning and appropriate referrals
  • Working closely with healthcare teams and multi-agency partners
  • Supporting service performance, outcomes and reporting requirements
  • Promoting equality, diversity and inclusive practice across all work

Experience in substance misuse, mental health or wellbeing services is required, along with a commitment to high-quality, person-centred support. Welsh and English speaking is preferred.

This is a chance to step into a senior position where you’ll directly influence recovery journeys while developing and leading a team. All prison-based roles are subject to enhanced DBS checks and HMPPS security vetting. Please note if you have lived overseas within the last 5 years then checks may take longer.
