This is a full-time role (35 hours per week), primarily based at HMP Swansea, with occasional travel to other sites across South Wales. This is a unique opportunity to support men in custody who are ready to make lasting change, achieve abstinence, and build a positive future beyond prison. If you’re committed to recovery, rehabilitation and making a real impact, we’d love to hear from you.
What You’ll Be Doing
- Deliver structured 1:1 and group interventions focused on substance misuse recovery
- Co-facilitate the Y Bont Programme across multiple cohorts each year
- Manage a caseload, assessments, care plans and progress reviews
- Use trauma-informed, harm reduction and abstinence-based approaches tailored to individual needs
- Work closely with prison staff, healthcare, probation, housing, employment and community partners
- Support release planning and continuity of care into the community
- Monitor outcomes, maintain accurate records and contribute to service targets
- Help organise graduation ceremonies and family engagement events
