We’re looking for a motivated and compassionate RECONNECT Worker to join The Forward Trust in a full-time role (35 hours per week) across HMP Wayland, HMP Norwich, and HMP Bure. You’ll play a vital part in supporting people as they transition from custody back into the community, helping reduce reoffending by ensuring individuals leaving prison are fully connected to the right health, wellbeing, and support services. This is a hands-on, relationship-driven role where you’ll work directly with service users pre- and post-release, building trust and helping to design and deliver personalised care plans that support long-term recovery and stability.

What you’ll be doing:

  • Carry out in-depth assessments and create tailored support plans for service users up to 6 months post-release
  • Act as a key link between prisons, community services, healthcare providers, and support agencies
  • Provide mentoring, advocacy, and practical support to help people engage with vital services
  • Manage referrals and ensure smooth, safe transitions into community care
  • Build and maintain strong partnerships with internal teams and external stakeholders
  • Support the recruitment, training, and development of mentors and peer supporters
  • Maintain accurate case records and contribute to performance reporting and KPIs

About you:

  • Full UK driving licence and access to your own vehicle (essential)
  • Confident working in a client-facing, multi-agency environment
  • Strong communication and relationship-building skills
  • Comfortable managing caseloads and working to targets
  • Passionate about supporting vulnerable people and reducing health inequalities
  • Able to work independently while contributing to a collaborative team culture

All prison-based roles are subject to enhanced DBS checks and HMPPS security vetting. These checks can take up to 5 months to complete. Any offer of employment will be conditional upon the successful completion of both checks.
