Date: 28th of August 2026 Are you passionate about making a real difference to people rebuilding their lives after prison?

We're looking for a compassionate and motivated RECONNECT Practitioner to support individuals before and after release, helping them access the health, wellbeing and community services they need to successfully reintegrate.

What you'll be doing Deliver personalised support to people preparing for release and for up to six months afterwards.

Complete holistic needs assessments and develop tailored support plans.

Build strong relationships with prisons, healthcare providers and community organisations.

Connect service users with GPs, mental health services, wellbeing support and local agencies.

Manage referrals, maintain accurate records and work collaboratively to achieve positive outcomes.

What we're looking for A caring, organised and proactive individual with excellent communication skills.

Someone who can build positive relationships with service users and partner agencies.

Experience supporting vulnerable people or working in health, social care, criminal justice or a related field is desirable.
