Looking for a role where you can genuinely change lives? We're looking for a passionate Health and Wellbeing Practitioner to join our team at HMP The Mount, supporting individuals on their journey towards recovery, better mental health, and a positive future beyond custody. This is a full-time position (35 hours per week).

In this rewarding and varied role, you’ll deliver person-centred support using both harm reduction and abstinence-based approaches. Working as part of an integrated healthcare team, you’ll support individuals experiencing substance misuse issues, low-level mental health needs, and wider health and wellbeing challenges, helping them build resilience and prepare for release back into the community.

What you’ll be doing:

  • You’ll play a key role in delivering high quality interventions and support, including:
  • Managing a caseload and delivering personalised support and care plans
  • Completing assessments, risk reviews and structured interventions
  • Delivering 1:1 sessions and group programmes on substance misuse and low-level mental health topics
  • Providing harm reduction advice and health and wellbeing information
