Health & Wellbeing Practitioner – HMP Aylesbury – Buckinghamshire
Permanent£24,000 – £28,647 depending on experience
Join Forward Trust as a Health and Wellbeing Practitioner at HMP Aylesbury and be part of a team that changes lives through substance misuse support and psychological care. This is your opportunity to work directly with individuals in custody, helping them move toward recovery, rehabilitation, and reintegration.
Working within an integrated healthcare framework, you’ll support adult men in prison (18+) with drug and/or alcohol misuse issues. You’ll deliver a range of psychosocial interventions, combining harm reduction and abstinence-based approaches, tailored to each individual.
As part of a client-centred service, your key responsibilities will include:
- Managing a caseload and conducting assessments and care planning.
- Delivering structured 1:1 and group sessions, including accredited treatment programmes.
- Providing harm reduction advice and relapse prevention support.
- Supporting throughcare planning in collaboration with partner services such as Probation, Housing, Employment, Healthcare, Gym, and transitional teams to prepare individuals for release and community reintegration.
- Occasionally acting as a second signatory or conducting oral swab testing to complement clinical services.

- Confident communicator with excellent record-keeping and IT skills.
