Are you passionate about helping people make positive changes in their lives? Join Forward as a Health and Wellbeing Practitioner, supporting adults aged 18+ with drug and alcohol issues within an integrated healthcare framework.
Youll work within our Health and Wellbeing Services, delivering psychosocial interventions and accredited treatment programmes for individuals in prison identified as having substance misuse problems. Every day will be different, but the goal is the same, empowering clients to build healthier, more stable futures.
What youll do:
Manage a caseload of clients, providing assessments, care plans and ongoing support.
Deliver 1:1 and group sessions, using both harm reduction and abstinence-based approaches.
Facilitate structured treatment programmes tailored to individual needs.
Provide harm reduction advice, education, and motivational support.
Work collaboratively with other services, including Housing, Employment, Gym, Healthcare, Probation, and external Drug Intervention Teams, to support throughcare and resettlement.
Complete second signatory and oral swab testing to complement clinical provision.
Contribute to local and Forward targets, focusing on measurable outcomes and service excellence.
