We're looking for a Health Wellbeing Practitioner to join our dedicated team, supporting adult and young men within the Criminal Justice System to improve their wellbeing, develop resilience and successfully reintegrate into their communities. What you'll be doing:

  • Working alongside experienced practitioners and partner organisations, you'll deliver personalised health and wellbeing support that empowers people to make meaningful, sustainable changes.
  • Providing tailored one-to-one and group interventions covering:
    • Family and significant relationships
    • Lifestyle and positive social networks
    • Emotional wellbeing
    • Social inclusion and community integration
  • Supporting individuals in custody to help them transition successfully back into the community, playing a vital role in every stage of their journey.
  • Managing your own caseload and building trusted relationships.
