- Working alongside experienced practitioners and partner organisations, you'll deliver personalised health and wellbeing support that empowers people to make meaningful, sustainable changes.
- Providing tailored one-to-one and group interventions covering:
- Family and significant relationships
- Lifestyle and positive social networks
- Emotional wellbeing
- Social inclusion and community integration
- Supporting individuals in custody to help them transition successfully back into the community, playing a vital role in every stage of their journey.
- Managing your own caseload and building trusted relationships.
