An excellent opportunity has arisen for a highly motivated, enthusiastic and caring person to join our Stourbridge District Nursing Team within Dudley NHS Foundation Trust, supporting Community Nursing staff. You will be part of a team providing high quality nursing care to patients in the community; a driver is essential.

Minimum training requirements include a qualification in NVQ Level 3 Health and Social Care. Previous experience as a Health Care Assistant in an Acute or Community Nursing setting is essential. You will have good communication skills and be able to work independently as well as part of a team.

Main duties of the job:

  • The post-holder is responsible for the ongoing assessment of care needs and the development, implementation, and evaluation of programmes of care.
  • The post holder will carry out all relevant forms of care and will take responsibility for caseload holders in their absence.

Job responsibilities:

Person Specification:

  • Proactive in learning and development of self, and show a willingness to retain information.
  • Experience as a Health Care Assistant in the acute or community nursing setting.
  • NVQ Level 3 in Health and Social Care.
  • English and Maths Qualifications at one of the following academic levels/grades: O Level - Grade A to C, CSE Grade 1 only, GCSE Grade A to C, GCSE Grade 9 to 4, Functional skills Level 2, AS/A Level, NVQ Level 3, Vocational Level 3 qualifications, or equivalent relevant knowledge.

Knowledge and Training:

  • Have some knowledge of NHS issues.
  • Show evidence of working to a plan of care.
  • A willingness to learn / competency in performing clinical observations including temperature, pulse, respirations, blood pressure, and oxygen saturations.
  • A willingness to learn / experience in the management of long-term conditions, for example, exacerbation of COPD.

Experience:

  • To show an ability to develop working relationships with other team members and professionals.
  • Able to comprehend instructions.
  • Evidence of being able to work as part of a team.
  • Knowledge / experience in diabetes care.

Communication & Relationship Skills:

  • The ability to keep all information confidential.
  • To be able to work independently in the home and provide feedback to other team members.
  • Able to communicate accurately in both verbal and written forms.
