Location: Allerdale House, Workington
Contract: Temporary (6 months)
About this opportunity
As a Trusted Assessor within a hospital-based service, you will work collaboratively with Case Managers, Discharge Navigators, and multidisciplinary partners across all Discharge to Assess pathways. The role focuses on supporting and facilitating safe, timely, and effective discharges from hospital into a range of health and social care services, including both community-based and building-based provision.
Working as part of an integrated team, you will triage and coordinate access to the most appropriate services and support following discharge. You will help ensure individuals achieve a seamless transition from hospital to community care.
