We’re looking for a dedicated and compassionate professional to join our Long-Term Assessment and Review Teams, where you’ll play a key role in delivering high-quality, person‑centred support. Every day, you’ll make a tangible difference, carrying out strengths‑based assessments, supporting safe and timely hospital discharges, and empowering individuals to live as independently as possible. This is a rewarding and varied role, where you’ll collaborate across health, social care, and community services to design support plans that create real, lasting impact.

You’ll bring a recognised Social Work qualification, professional registration, and solid post‑qualification experience.
