Reablement Support Workers will work with individuals requiring reablement services to achieve their goals and promote independence. A valid driving licence is essential for this role.

Job Accountabilities

  • Meet reablement goals/outcomes as outlined in the care plan.
  • Identify and support additional goals that may enhance quality of life and independence.
  • Provide feedback on progress and goals to the Reablement and Assessment Officer.
  • Maintain clear and timely communication regarding individual progress.
  • Collaborate effectively within a small team to ensure consistent support and address concerns.

Additional Information

This role requires an Enhanced DBS Check with an adults Barred List check. Working in regulated activity with adults, applicants barred from such activities are ineligible.
