Reablement Support Workers will work with individuals who have reablement needs to help them achieve their goals, promoting independence and improving quality of life. A valid driving licence is essential.
Job Accountabilities:
- Ensure reablement goals/outcomes are met according to the reablement plan.
- Identify additional goals/outcomes that could enhance quality of life and independence during the reablement episode.
- Provide feedback on additional goals/outcomes to the Reablement and Assessment Officer.
- Maintain clear and timely communication with the Reablement and Assessment Officer regarding progress.
- Work closely within a small team of reablement support workers, communicating effectively about progress or concerns.

Additional requirements
This role requires an Enhanced DBS Check with an adults Barred List check. It involves working in regulated activity with adults. It is an offence for barred individuals to apply for roles involving unsupervised work with adults.
