Location: Penrith, Kendal orBarrow-in-Furness

ContractType: Permanent

We are looking for an experienced,person-centred Occupational Therapist to join our Adult SocialCare Reablement Team in Westmorland and Furness.

This is a rewarding role which is centralto our Promoting Independence Well-being Programme, which focuseson early intervention and recovery – helping people stay incontrol of their lives and reduce the need for long term care.You will support people to remain independent in their own homesor return home safely after a hospital stay. You’ll helpindividuals regain confidence and live the life they want to,doing the everyday things that matter to them.

What the roleinvolves:

  • Visiting people in their own homes toassess their needs
  • Completing Care Act and functionalassessments
  • Creating support plans that promotesafety, independence and well-being.
  • Recommending equipment or adaptations toreduce risk and support mobility
  • Working closely with social workers,reablement workers, health professionals and families.
  • Supporting people with a wide range ofphysical, cognitive, mental health and social needs.

Who we arelooking for

We are looking for a qualified OccupationalTherapist (HCPC Registered) with experience of working with adultsocial care, reablement, community or hospital dischargesettings.

You will need strong communication andproblem-solving skills with the ability to focus onperson-centred, strength- based practice.

The role will require willingness to travelacross Westmorland and Furness area.

Unfortunately, we are unable to acceptnewly qualified Occupational Therapist for this post.
