Role summary
We are looking for an experienced, person-centred Occupational Therapist to join our Adult Social Care Reablement Team in Westmorland and Furness. This is a rewarding role central to our Promoting Independence Well-being Programme, focusing on early intervention and recovery – helping people stay in control of their lives and reduce the need for long term care. You will support people to remain independent in their own homes or return home safely after a hospital stay. You’ll help individuals regain confidence and live the life they want to, doing the everyday things that matter to them.
What the role involves
- Visiting people in their own homes to assess their needs
- Completing Care Act and functional assessments
- Creating support plans that promote safety, independence and well-being
- Recommending equipment or adaptations to reduce risk and support mobility
- Working closely with social workers, reablement workers, health professionals and families
- Supporting people with a wide range of physical, cognitive, mental health and social needs
Who we are looking for
We are looking for a qualified Occupational Therapist (HCPC Registered) with experience of working with adult social care, reablement, community or hospital discharge settings. You will need strong communication and problem-solving skills with the ability to focus on person-centred, strength-based practice. The role will require willingness to travel across Westmorland and Furness area.
