The Confederation, Hillingdon CIC is seeking an experienced General Practice Nurse to join North Connect PCN in Northwood on a part-time contract (25 hours/week). The post holder will work autonomously to provide high-quality, person-centred nursing care for patients with long-term conditions and complex needs, within enhanced practice and PCN settings.
Responsibilities include care planning, self-management support, timely referrals, and leadership within the nursing workforce.
