The Confederation, Hillingdon CIC is seeking an experienced registered nurse to work at enhanced practice level across the Primary Care Network in Colne Union PCN (Yiewsley/West Drayton). You will deliver person-centred care for adults with mental health and learning disabilities, using advanced assessment skills and evidence-based decision making, and you will contribute to service development with GPs, community teams and other partners.
