About Us:
The Confederation, Hillingdon CIC works with General Practice and other healthcare providers to deliver its vision for “Hillingdon to deliver the best primary care outcomes for patients in the whole of London”. We are a not-for-profit community interest company. The Confederation works to develop and support individual GP practices, PCNs and Neighbourhoods and their changing needs. We deliver excellent clinical services ourselves both at scale and complementary to General Practice. We are the provider representative voice for local General Practice into the wider NHS and other Partners. We are ‘of the NHS’ but independent, innovative and transformational.
The Confederation determines to develop as an attractive place to work, providing rewarding roles and opportunities to grow in order to attract and retain great staff that in turn delivers our vision.
Our Values:
Organisational Summary - HHCP
Hillingdon Health and Care Partnership (HHCP) brings together local health and social care providers to deliver coordinated, person-centred services in the community.
The Reactive Care Coordination Hub forms part of HHCP’s urgent and proactive care model, providing rapid clinical assessment, crisis response, and short-term interventions to avoid unnecessary hospital admissions. The Hub works collaboratively with the Urgent Community Response (UCR) team, Virtual Ward, Discharge to Assess (D2A) services,
Your Life Line (YLL), primary, community, secondary care, local authority and voluntary sector partners.
The Reactive Care Programme is improving urgent and crisis care so residents receive the right care, in the right place, at the right time when health or social care needs escalate.
It brings together our Integrated Urgent Response, hospital avoidance services, and discharge support – creating a single pathway for unplanned care outside of the acute hospital.
The goal is a 24/7 community-based urgent care system able to respond to crises within two hours, deliver short-term treatment and monitoring at home, and coordinate a smooth return to routine or planned care.
This transformation is vital to:
- Reduce pressure on A&E and 999
- Prevent unnecessary admissions
- Enable timely discharges
- Improve patient outcomes and system flow
