The Confederation, Hillingdon CIC is recruiting a full-time Clinical Pharmacist to join North Connect PCN at Carepoint Practice in Northwood. You will work closely with GPs and be part of a team of Clinical Pharmacists delivering safe prescribing and enhanced patient care.
You will develop repeat prescription authorisation processes, manage medicines on transfer of care, and engage with patients to assess and treat conditions, including long-term care and promoting self-care in line with NICE
