We are a not-for-profit community interest company. The Confederation works to develop and support individual GP practices, PCNs and Neighbourhoods and their changing needs. We deliver excellent clinical services ourselves both at scale and complementary to General Practice. We are the provider representative voice for local General Practice into the wider NHS and other Partners. We are ‘of the NHS’ but independent, innovative and transformational.

We have a new and exciting opportunity to appoint nurses to join our dynamic and friendly operations team. The post-holder will have the opportunity to work across different primary care services. You will be dealing with a wide range of patient health needs across the local area. You will be supported in your role through the clinical leads across the services you are working in as well as an experienced and friendly management and administration team. This post may require you to travel between sites within the borough of Hillingdon as and when required. You must be registered with the Nursing and Midwifery Council (NMC) and hold pin and a certificate of completion of training.

Primary Responsibilities:

• In accordance with the services needs the post-holder will make him/her-self available to undertake a variety of duties including cervical smear tests, post-operative dressings, HRT reviews, Pill Checks, Immunisations, BP checks, asthma checks and diabetic reviews and spirometry if trained.
• Assessing the health needs of patients with chronic health needs and using your own judgment to assess whether further input is needed.
• Screening patients for disease risk factors and early signs of illness.
• Contributing to QOF requirements and recording data appropriately.
• Contributing to PCN requirements and recording data appropriately.
• In consultation with patients and in line with current practice disease management protocols, developing care plans for health related illnesses.
• Screening patients for health education.
• Admitting and discharging patients to and from the caseload and referring to other care providers as appropriate.
• Recording clear and contemporaneous consultation notes to agreed standards.
• Collecting data for audit purposes and contributing to the practice audit programme as required.
• Take responsibility for some aspects of the clinical work within the team, for example: coordinating aspects of governance, QOF, enhanced services, CPD and keeping clinical protocols up to date.
• In general, the post holder will be expected to undertake all the normal duties and responsibilities associated with a practice nurse working within primary care, as set by Royal College Nursing and the Nursing and Midwifery Council and councils applicable to the staff within the practice, Department of Health, Clinical Commissioning Groups and all other regulatory and standard setting organisations.

• Essential Criteria:
• Eligible to work in UK
• Able to carry out duties with or without premises adaptation
• NMC registration
• Experience of working within the NHS, with general public & team working
• Well developed IT skills and an enthusiasm for new applications and software
• Good time management and highly organised
• Ability to use initiative, judgement, resourcefulness, common sense and local knowledge
• Interest in quality improvement work
• Able to obtain and analyse complex technical information
• Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors when appropriate
• Able to work under pressure and to meet deadlines
• Experience working within primary care
• Certificates for Practice Nurse Training and cervical Smears Training with relevant registration numbers.
• Work effectively independently and as a team member
• Demonstrates accountability for delivering professional expertise and direct service provision
• Self-Motivation Adaptable Immunisation status - Hep B / MMR
• Desirable Criteria:
• Other postgraduate qualifications (Minor Surgery, Education, Family planning etc)
