Overview

We’re excited to offer an opportunity for a skilled Diabetes Assistant Practitioner or Nursing Associate to join our Integrated Community Diabetes Service. In this pivotal role, you will provide crucial support and continuous education to individuals living with diabetes, helping to enhance their care and quality of life.

You will deliver care and support to adults with diabetes across a spectrum of settings, including MDT clinics, group education sessions, and one-to-one interactions. We are seeking a candidate with a diverse skill set, particularly in venepuncture, foot surveillance, vital signs monitoring, and providing assistance in clinical environments. Experience in health or social care or primary care is relevant.

Location flexibility includes rotation through various clinic locations and home visits within the Heywood, Middleton, Rochdale, and Bury boroughs. You will be under the guidance and supervision of Specialist Diabetes Nurses/Dietitians and will have opportunities to grow and develop within the team.

This full-time role is Monday to Friday, with the option of compressed hours. The Northern Care Alliance NHS Foundation Trust (NCA) provides hospital and integrated health and social care services to over one million people in Greater Manchester, with colleagues across Bury, Rochdale, Oldham and Salford.

Responsibilities

  • Collaborate with the community diabetes multidisciplinary team to provide holistic care to patients with diabetes.
  • Support the clinical team with educational group sessions and contribute to individual patient care plans.
  • Rotate through various clinic locations and conduct home visits within the defined boroughs.
  • Assist in clinical environments, including venepuncture, foot surveillance, and monitoring vital signs, under supervision.

Qualifications and Requirements

  • Health and Social Care (Assistant Practitioner) FdSc or Health and Social Care Foundation Degree (Fda) or equivalent is essential.
  • Experience or competence in venepuncture, foot surveillance, vital signs monitoring, and general clinical support.
  • Ability to work across multiple settings, including clinics, education sessions, and home visits.
  • Full-time role with Monday–Friday schedule; option of compressed hours if offered.
