The Children's Community Nurse (CCN) is a member of the Children's Community Nursing Team providing specialised and skilled nursing care of children in the community as designated by the BCYP Operational Lead/Team Leader of the CCN service. To reduce unnecessary hospital admission and promote hospital avoidance and to provide specialist nursing support in the child’s own environment in partnership with the child, young person, family, carers and other professionals.
