Chesser Surgery is seeking a motivated and experienced Practice Nurse to join our team in Carshalton, Surrey.
Responsibilities
- Conduct comprehensive assessments of patients’ health needs.
- Develop, implement and review individualized care plans.
- Provide appropriate treatments and interventions as required.
- Undertake treatment room responsibilities and deliver Chronic Disease Management (CDM) clinics.
- Carry out clinical tasks such as travel health consultations and vaccinations, cervical screening tests, childhood immunisations, blood pressure monitoring, ECGs, venepuncture and routine health checks.
- Work collaboratively with GPs, fellow nurses and the wider healthcare team to ensure high‑quality patient care.
- Maintain accurate, timely and confidential patient records in line with NHS standards.
- Deliver health education, advice and counselling to patients and families.
- Keep up to date with current clinical guidelines, medical advancements and best practice in primary care.
- Work part‑time (approximately 16 hours per week), with flexible arrangements available for the right candidate.
Qualifications
- Registered Nurse (NMC registration) with evidence of current NMC revalidation.
- Experience in primary care or a transferable clinical setting.
- Competence in immunisations and vaccinations (adult and childhood).
- Cervical cytology training.
- Wound care and dressing experience.
- Ability to manage Chronic Disease Management (CDM) clinics, e.g. diabetes, asthma, COPD, hypertension.
- Good IT skills and ability to use electronic patient records.
- Previous experience working as a Practice Nurse in General Practice.
- Travel health training and vaccination experience.
- Additional qualifications in chronic disease management (e.g. diabetes diploma, respiratory training).
- Evidence of ongoing professional development (CPD courses, certificates).
- Mentor/Student supervision and assessment (SSSA) qualification for supporting students or junior staff.
