Central Surgery is based in Barton-upon-Humber, with a branch site in the village of Goxhill. We are looking to recruit a clinical pharmacist to join our growing MDT team. Ideally candidates will hold an independent prescribing qualification, but we will support the successful candidate in achieving this qualification. The clinical pharmacist is a patient-facing role and there will be plenty of opportunity to utilise your clinical skills to improve the health of the local community.
Main duties of the job:
- The successful candidate will work autonomously within the practice, but also as part of the surgery team, to deliver the safe and effective use of medicines through managing long-term conditions, supporting hospital discharge prescribing arrangements, providing specific advice for those on multiple medications.
- You will act as a specialist resource to answer medicines-related queries, applying your clinical and pharmaceutical skills to a variety of tasks within the medicines optimisation agenda.
- You will work alongside our highly valued existing practice and network clinical pharmacists as the role develops to meet the NHS enhanced service requirements.
- The successful candidate should be enthusiastic and adaptable to change, with excellent clinical skills and a good level of competence in the use of IT.
- Good organisational skills and the ability to communicate effectively with a wide range of healthcare professionals and other stakeholders including patients, families and carers is an advantage.
About us:
We are a large semi-rural dispensing practice, with 2 sites, serving our local community of approximately 17,000 patients. Rated as good by the CQC, we consider our surgery to be a dynamic, forward-thinking practice. Advancing in times of change we operate a total triage system led by GPs. Working with the PCN, we provide care aligned to both national and local enhanced services, achieving high results for both QOF and IIF.
We are looking for a clinical pharmacist to join our multi-disciplinary team; someone who will help deliver a high level of patient care and be a key member of staff, undertaking both clinical and educational duties if needed.
Job responsibilities:
- The post holder will be supported by existing clinical pharmacists employed within the practice and a GP clinical mentor.
- You will be expected to lead in developing, promoting and implementing the high quality, evidence-based and cost-effective use of medicines in primary care in a way that maximises the benefits of medicines to patients and which minimises risks (clinical, legal and financial).
- The post holder will also be required to develop, support and implement agreed prescribing and pharmaceutical activities or programmes within the practice.
Key Responsibilities:
- Patient-Facing Clinical Medication Reviews.
- Undertake clinical medication reviews with patients (telephone and face to face), working within your scope of practice as a non-medical prescriber, implementing any necessary changes. The reviews could be cohort based, in care homes, polypharmacy or any other area as required, within your level of competence.
- To participate in, and coordinate medicines optimisation services and clinical pharmacy development, to improve quality of care, personalised care and patient experience.
- Provide clinical leadership to ensure high quality and cost-effective prescribing.
Person Specification:
- Understanding of the clinical pharmacist role within primary care.
- Demonstrates the ability to communicate complex and sensitive information in an understandable form.
- Work effectively independently and as a team member.
- An understanding of primary care prescribing.
- Experience of working in Primary Care.
- Experience in the use of various IT applications and the recording of clinical notes (SystmOne, AccuRx).
Qualifications:
- Completed CPPE primary care pathway.
- Two years post qualification experience.
