JOB TITLE: Licensed Practical Nurse (LPN) / School Nurse

DEPARTMENT: Nursing

REPORTS TO: Nurse Director

PURPOSE: Licensed Practical Nurses regardless of specialty or work setting, provide practical nursing care in conjunction with the Registered Nurse. The LPN is a core member of the interdisciplinary care team for the children and adults we support with Complex Medical Conditions. The LPN is guided by the Nurse Practice Act and the Code of Conduct for Nurses. The LPN School Nurse will utilize the nursing process, critical thinking, and problem solving skills to provide holistic care to the students we support across the life span.

Functions And Responsibilities

  • Safety & Well-Being of Students: Provide ongoing observation of students within the framework of holistic nursing practice and in accordance with the principles of individual and family centered care. Observe and report data regarding individual and families’ culture, value system, lifestyle and the interest and ability to participate in their care, and any other factors impacting successful treatment outcomes.
  • Student Health Planning: Develop and implement a comprehensive person specific nursing plan of care in collaboration with the Registered Nurse and aligning with The Center for Discovery Philosophy. Coordinate and update an individualized plan of care, based on evidence in collaboration with the Registered Nurse, the individual, their family, and the interdisciplinary team promoting continuity of care.
  • Intervention: Establish and maintain a therapeutic relationship with students and families. Identify, initiate and coordinate therapeutic interventions consistent with the scope of practice and level of competence. Implement prescribed interventions and perform independent nursing interventions.
  • Evaluation: Ensure that students plan of nursing care is appropriate based on relevant data and evidenced based best practices. In conjunction with the Registered Nurse; utilize data obtained from individual, family, and other health care providers as well as internal data collected to evaluate the plan of care including individual responses to interventions and achievement of expected outcomes.
  • Professional Development and Accountability: Practice within the professional, legal and ethical standards and is accountable for his or her own actions and decisions at all times. Demonstrate professionalism in attitude, dress and behavior.
  • Professional Leadership, Education and Research: Support the philosophy, vision and mission of The Center for Discovery, including the HealthE6 framework. Promote a safe work environment by identifying and reporting potential risk issues.

Qualifications:

  • Graduate of an approved Practical Nursing Program.
  • Two years of work experience in nursing preferred with expertise based on prior experience with students with intellectual or physical disabilities.
  • Demonstrate skill and expertise in utilization of the nursing process and interdisciplinary team approach.
  • Maintain required licensing and mandated training by the NYS Education Department.

Licensure/Certification:

  • Licensed Practical Nurse with current licensure in New York State.
  • Current Basic Life Support Certification.

  • Ability to work within the team environment and to actively contribute to individual care planning.
  • Demonstrate strong communication skills and ability to adapt communication styles to meet the needs of the individual, family or healthcare team member.
  • Ability to safely administer medications and respiratory treatments.
  • Demonstrate knowledge of the roles, responsibilities and rights of other nurses and other healthcare team members.

PHYSICAL DEMANDS:

  • Frequently on feet, walking and standing required.
  • Frequently required to perform a two person lift and/or transfer.
  • Required to lift equipment or materials of various weights and sizes as needed.

WORK ENVIRONMENT:

  • Exposure to wet and/or humid conditions.
  • Exposure to fumes or airborne particles.
  • Exposure to human elements/excrements.
  • Noise level in the work environment is usually loud.
  • Exposure to outdoor weather conditions.
