Purpose: Licensed Practical Nurses, regardless of specialty or work setting, provide practical nursing care in conjunction with the Registered Nurse. The LPN is a core member of the interdisciplinary care team for the children and adults we support with Complex Medical Conditions. The LPN School Nurse will utilize the nursing process, critical thinking, and problem solving skills to provide holistic care to the students we support across the life span.

Responsibilities:

  • Provide ongoing observation of students within the framework of holistic nursing practice and in accordance with the principles of individual and family centered care.
  • Observe and report data regarding individual and families' culture, value system, lifestyle and the interest and ability to participate in their care, and any other factors impacting successful treatment outcomes.
  • Identify and communicate individual and family needs to the supervising Registered Nurse, interdisciplinary health care team, and other providers as needed.
  • Provide ongoing monitoring of individual response to intervention.
  • Identify and report deviations from expected outcomes and change in individual status to the supervising Registered Nurse.

Student Health Planning:

  • Coordinate and update an individualized plan of care, based on evidence in collaboration with the Registered Nurse, the individual, their family, and the interdisciplinary team promoting continuity of care across the continuum.
  • Prioritize nursing interventions to achieve individual outcomes and goals.
  • Promote a comprehensive individual plan, coordinated by the interdisciplinary team to meet the individual's needs, including health promotion, self-management, and coordination of resources.
  • Update student's medical records as required, including updated immunization records and providers orders in compliance with NYS Education Department requirements.
  • All records should be provided to the School Nursing Director in a timely manner.

Interventions and Evaluation:

  • Establish and maintain a therapeutic relationship with students and families.
  • Identify, initiate and coordinate therapeutic interventions within scope of practice.
  • Implement prescribed interventions and perform independent nursing interventions.
  • Consult with the Registered Nurse, interdisciplinary team and medical home providers to discuss changes and modify interventions.
  • Provide information and instruction to assist individuals to understand health care practices and processes.
  • Care is coordinated within available resources of The Center for Discovery.

Professional Development and Accountability:

  • Practice within professional, legal and ethical standards; be accountable for actions and decisions.
  • Demonstrate professionalism in attitude, dress and behavior.
  • Contribute to positive team functioning and cultural competence.
  • Engage in ongoing professional development and quality improvement.
  • Report professional practice issues according to Center for Discovery standards.

Qualifications:

  • Graduate of an approved Practical Nursing Program.
  • Two years of nursing experience preferred, with expertise related to students with intellectual or physical disabilities.
  • Experience with the nursing process and interdisciplinary team approach.
  • Maintain current NYS licensure and required trainings.

Licensure/Certification:

  • Licensed Practical Nurse with current NYS licensure.
  • Current Basic Life Support Certification.

Personal Characteristics:

  • Ability to balance and prioritize multiple demands.
  • Strong communication and teamwork skills.
  • Ability to safely administer medications and respiratory treatments.
  • Proficiency with Microsoft Word/Excel and Electronic Health Records (EHR) systems.
  • Critical thinking, sound judgment, and adaptability.

Physical Demands:

  • Frequent standing, walking, and lifting as required.
  • Ability to assist with transfers and equipment as needed.
  • Donning of PPE as required.

Work Environment: Exposure to various environmental conditions and potential exposure to contagious diseases.
