Support Worker – Community & Home-Based Support (Warwickshire)
We are looking for caring, reliable, and flexible Support Workers to join our growing team, providing person-centred support to individuals within their own homes and local communities across North and South Warwickshire.

Key Responsibilities
- Providing personal care and support with daily living activities.
- Supporting individuals with medication administration and management.
- Assisting with community access, appointments, social activities, and hobbies.
- Encouraging independence and promoting positive outcomes.
- Maintaining accurate records and reporting any concerns appropriately.
- Working in line with care plans, risk assessments, and organisational policies.
What We're Looking For
- A full UK driving licence and access to a vehicle.
- Experience supporting individuals in a variety of care settings, with a minimum of 12 months of experience.
- Genuine passion for building positive relationships and helping people achieve their goals.
- Competence and confidence in administering medication.
- Experience providing personal care.
- Excellent communication and interpersonal skills.
- A flexible approach to working across different locations and support packages
- The ability to provide consistent support and develop trusting relationships with the people you support.
A commitment to delivering person-led support that focuses on individuals' choices, independence and well-being.
Working Pattern
- Availability to work a variety of shifts across the week, including weekdays and weekends.
- Opportunities for sleep-in shifts may also be offered.
- Travel required across North and South Warwickshire.
