Poole North Primary Care Network (a network of four GP practices working together to cover a patient population of 52,000 patients) are looking for an Advanced Nurse Practitioner to work from the Hadleigh Practice (at Broadstone and Corfe Mullen) for 20 hours per week. The practice is a busy, yet friendly GP practice of 20,000 patients who put patients at the very heart of everything they do. The role comes with supervision from a GP partner and networking opportunities with ANPs in the other practices.
Hours of work
- Wednesday 8am to 6.30pm (with half an hour unpaid break)
- Friday 8am to 6.30pm (with half an hour unpaid break)
Main duties of the job
The post-holder will manage a caseload and deal with a wide range of health needs:
- Act as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients attending within primary care initiating direct referrals as appropriate in a variety of appointment types (face to face, telephone, video).
- Assess, diagnose and treat patients attending primary health care services with a range of acute, non-acute and chronic medical conditions and refer appropriately in line with local and national guidance.
- Admit patients to secondary care for acute medical need.
- Refer patients as appropriate to secondary care including referral for x-ray in line with policy and appropriateness.
- Order blood and other pathology diagnostic testing via ICE.
- Work within all relevant practice policies and procedural guidelines e.g. infection control, chaperoning, risk management.
- Prescribe independently but in line with the prescribing formulary.
- See patients from the age of 3 up excluding patients who are pregnant, working within competencies but mostly seeing all acute patients apart from those with pregnancy/gynae related issues or mental health acute needs.
- Sign fit notes when required.
- Undertake appropriate home visits if required.

The Hadleigh Practice based in both Broadstone and Corfe Mullen puts patients at the heart of everything they do and are a busy but friendly and supportive organisation who are part of Poole North Primary Care Network. Together we have, for the last six years, been able to ensure our patients have access to additional services which otherwise would not be available to them and we work together to achieve the highest standard of care. This role will have access to a GP partner who will act as your supervisor for regular supervision sessions and the other network ANPs can come together for peer support.

Person Specification
Qualifications: The post holder will be required to be a fully qualified and registered Advanced Nurse Practitioner and hold an MSc in Advanced Practice, and/or have a Digital Badge (or be willing to work towards one). Additionally, you must be an independent prescriber.
