Salary: £24,479 per annum pro-rata (£13.45 per hour)
Driving Requirement: A full UK driving license held for a minimum of 2 years is required and access to a vehicle. Business mileage reimbursed.
- Managing a caseload to provide emotional and practical support for our service users who have been discharged from hospital including welfare calls, shopping, and home visits.
- Supporting adults after a hospital stay to help them recover safely at home.
- Responding to referrals from clinicians and community health professionals.
- Assessing needs and following a personalised support plan for each service user.
- A kind, calm and confident approach with great people skills.
- The ability to work independently and as part of a team.
- A full UK driving licence and access to a vehicle
- Basic IT skills and a commitment to following procedures.
