Adult Community Clinical Services is a Department of Mental Health funded service that is designed to deliver evidence-based interventions within a clinically focused model. Individuals served are diagnosed with mental illness or dual diagnosis and reside in their own homes, apartments, or in group living environments. Our services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals. The teams are tight knit, supportive and multidisciplinary. Open Sky’s talented clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff provide quality, compassionate care to the individuals served.
Responsibilities
- Residential Counselors support individuals to live as independently as possible, while providing evidence-based treatment and care within a group living environment.
- Provide guidance, support, and perform various levels of activities of daily living.
- Implement procedures that ensure the health, safety, and well-being of individuals and general cleanliness and safety of the physical site.
- Assist in developing individualized, person-centered treatment plans and play a lead role in the daily implementation plans.
- Assist individuals to access and/or provide transportation.
- Implement activities that create opportunities for the development of valued roles and personal relationships in the community.
- Review and complete all required documentation.
Qualifications
- High School Diploma, GED or equivalent, required.
- Valid Driver’s License and acceptable driving record, required.
- Reliability, willingness to learn, and being open to new opportunities.
