The post holder is responsible for assessing, coordinating, developing, implementing, and evaluating care programmes for adults with mental health problems referred to the First Response and Recovery and Wellbeing Team. They report daily to the Clinical Lead and are ultimately accountable to the Team Manager for the overall service. The post involves working within a multidisciplinary team, collaborating closely with GPs, wards, and primary healthcare professionals. The post also includes maintaining standards of care within the community mental health service. The post holder must ensure the delivery of an integrated service to clients and foster a good interdisciplinary working environment.

KEY RESPONSIBILITIES
  • Manage patient caseloads: Provide effective individual care addressing mental, health, and social needs through holistic assessment, planning, implementation, and evaluation of interventions. Maintain clear, accurate, and concise documentation of client care.
  • Initial assessment and liaison: Participate in initial client assessments to determine suitability for the Community Mental Health Service. Liaise with referring agencies to identify alternative services as needed.
  • Team participation: Engage in team and clinical meetings, contribute to discussions on new care models, and support the introduction of research or trials.
  • Risk management: Understand the importance of effective risk management, conduct clinical risk assessments, and monitor clients' mental states to evaluate risks to themselves or others.
