The Advanced Nurse Practitioner is responsible for providing senior clinical leadership and advice within the Home First Team, which offers crisis and intensive support to patients. The role involves managerial input, clinical leadership, and daily coordination of clinical practice to ensure effective care across the Urgent Care Pathway.
You will collaborate with key partner organizations, service users, carers, clinicians, and other practitioners within a multidisciplinary team to deliver a 24/7 service, including participating in projects to review and develop service provision. Lead the ongoing development of a high-quality, evidence-based service aligned with care pathways, NICE guidance, and Department of Health (DH) guidelines. This includes service development and policy work in conjunction with other senior colleagues.
You will also oversee the supervision and management of lower banded staff within your team, in accordance with Trust HR policies. We seek motivated staff who share our Trust values of Care, Learn, and Empower.

Key Responsibilities:
- Proactive assessment, monitoring, and management of patients, ensuring care is evidence-based and standards-compliant.
- Providing clinical leadership in service evaluation and monitoring.
- Performing specialist assessments as required.
- Undertaking and supervising nursing needs assessments and carers assessments in line with Trust KPIs.
- Using relevant screening and assessment tools, supervising staff in their use.
- Leading comprehensive assessments, care planning, and reviews aligned with mental health care clusters.
- Formulating, delivering, and reviewing individualized, evidence-based care packages according to NICE guidance, including non-medication strategies.
