Main duties of the role include:
Providing clinical care in the treatment room, including wound care, injections, and immunisations.
Supporting the management of long-term conditions such as diabetes, asthma, and hypertension.
Performing routine health checks, screenings, and vaccinations.
Working collaboratively with the Lead Practice Nurse, GPs, and the wider multidisciplinary team.
Contributing to the development and improvement of primary care services in response to increasing demand.
We are looking for individuals with proven Primary Care nursing skills and experience, who can demonstrate safe, evidence-based clinical practice, excellent communication, and a commitment to patient-centred care.
Main duties of the job
Provide high-quality nursing care to patients in a primary care setting, including managing long-term conditions such as diabetes, asthma, and hypertension.
Conduct routine health checks, screening, immunisations, and travel vaccinations, as well as treatment room procedures including wound care and minor procedures. Support patient self-management through education, advice, and health promotion.
Work collaboratively with GPs, the Lead Practice Nurse, and the wider multidisciplinary team to ensure safe, effective, and patient-centred care.
Contribute to service development and quality improvement initiatives, helping to shape and improve the patient experience at the practice.

We are seeking a Practice Nurse with experience in a general practice setting to join our friendly and supportive clinical team. We welcome applications from candidates with secondary care or community nursing experience as well.
As a key member of our Nursing Team, you will provide high-quality, patient-centred care, managing on-the-day consultations and supporting patients with a wide range of minor illnesses and long-term conditions. You will carry out thorough assessments, deliver treatment and interventions, and proactively help to reduce delays in patient care.

The surgeries are located approximately 2 miles apart (around a 5-10 minute drive depending on traffic), ensuring close collaboration and easy access between sites.
Person Specification
Qualifications
- Bachelors degree in Nursing or an approved nursing qualification.
- Registered with the Nursing and Midwifery Council (NMC).
- Current professional registration and eligibility to practice in the UK.
- Non-medical prescribing qualification (Level 6 or 7) for Health Care Professionals.
- Additional qualifications in areas such as immunisation, wound care, or long-term condition management.
Experience
- Experience working in a general practice or primary care setting.
- Excellent knowledge of core nursing practice and patient care principles.
- Skilled in managing long-term conditions (e.g., diabetes, asthma, hypertension).
- Experience providing immunisations, health checks, and routine treatment room procedures.
- Strong communication and interpersonal skills with patients and colleagues.
- Ability to manage own workload and work autonomously under supervision.
- Empathy, sensitivity, and professionalism in patient care.
- Specialism in areas such as, but not limited to, respiratory, cardiovascular, diabetes.
- Experience with health promotion, patient education, and chronic disease management programs.
