Type: Fixed term maternity cover, 35 hours per week

Annual leave: 25 days, increasing to 30 days after five years

What youâll be doing
The role supports people in prison to navigate complex health and social care, removing barriers to selfâmanagement and embedding coping strategies through individualised and group interventions.
Main Duties
- Work with a variety of people in prison in oneâtoâone and group settings, using peer support coaching interventions and evidenceâbased tools to help them achieve goals.
- Develop and maintain good communication with service users to ensure strengths, goals and preferences are met, and connect them to relevant services.
- Manage a caseload and record and report activity timely.
- Develop and maintain positive relationships with key prison and communityâbased service providers to ensure effective onward referrals.
- Undertake personâcentred assessments to identify desired changes or recovery, assess risk to self or others.
- Make decisions on suitability of new referrals; support the person to the right part of the mental health pathway within prisons.
- Build up knowledge of available services, and assist with recording this locally and centrally.
- Work with client groups to help them achieve goals, identify local gaps and feed information into team meetings.
- Support and assist with monitoring and evaluating the programme, including collating relevant monitoring information and collecting qualitative outcome data from case studies, focus groups and interviews.
- Contribute to development of the service, including quality assurance and improvement systems.
- Ensure quality standards are adhered to and statutory responsibilities are followed and reported as required.
- Participate in caseload management with line manager or coordinator.
