We are looking to recruit an enthusiastic qualified Advanced Practitioner/Advanced Nurse Practitioner with prescribing to come and join our lovely team in Bradford City. We offer support and mentorship as well as the right level of autonomy. The position is for 4-6 sessions per week/2-3 days ideally but this is negotiable for the right candidate. We have been awarded Gold award Investors In People, CQC Good rating and we are high QOF achievers. We offer opportunities for development and special interests and are looking for someone who will fit into our current team who work closely together and with our Salaried GPs.
Main duties of the job
- To deliver a high standard of patient care as an Advanced Nurse Practitioner (ANP)/Advanced Clinical Practitioner in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base.
- Telephone triage, face to face clinics, pathology management and development of team and peer support.
- Prescribing efficiently and effectively within boundaries.
- To manage a clinical caseload, dealing with presenting patients' needs in a primary care setting.
- To provide clinical leadership within the practice.

- To manage own clinical workload in general practice responding effectively to patient need and ensuring patient choice and ease of access to services.
- To be available to mentor and support other nurses and health care assistants in developing and maintaining clinical skills.

We are also a training practice for Trainee Nurses.
We are consistently high QOF achievers, CQC Good (July 2019), Investors in People Gold Award achievers (December 2022 and reaccredited in 2025) and participate in numerous Enhanced Services including Anticoagulation, Diabetes, Ultrasound services, Sexual Health and Dermatology.
Job responsibilities
- To work in partnership with the Primary Health Care team (PHCT) in providing patient-focussed care responsive to health need.
- To work in partnership with the PHCT in developing new services for patients.
- To utilise the skills / knowledge and competencies required in order to provide first point of contact direct access for patients with undifferentiated and undiagnosed problems seeking a primary health care service.
- To utilise the skills / knowledge and competencies required in order to be responsible and accountable for assessing patients with undifferentiated and undiagnosed health care needs.
- To utilise skills / knowledge and competencies required in order to be responsible and accountable for managing a caseload of patients, for treating, referring, admitting and discharging patients as appropriate following training.
- To utilise skills / knowledge to make judgments involving complex facts or situations, which require analysis, interpretation and comparison of a range of options, which could be complex and made up of several conflicting components and where expert opinion may differ or not all information is available.
- To utilise skills / knowledge and competencies required in order to be responsible and accountable for managing the nursing team.
- To utilise skills / knowledge and competencies required in order to be responsible and accountable for achievement of Quality and Outcome Framework (nGMS GP contract) targets set by Avicenna Medical Practice.
Person specification
- RN 1st Level Current NMC Registration Registered nurse prescriber.
- Understanding of NHS Plan/NSFs and implications for Primary Care.
- Evidence of having or willing to undertake advanced education and training in clinical examination, history taking, therapeutic communication, pharmacology, pathophysiology etc.
- Evidence of continuing professional development.
- Minimum of 2 years post-registration experience of working in primary/community/acute health care setting.
- Minimum of 1 year experience of managing the care of patients in the primary care setting.
- Experience of providing effective health education and health promotion to patients presenting with a range of health needs.
- Completed Nurse Practitioner (Primary Care) Post Graduate level Qualification in teaching and assessing.
- Understanding of National Nursing strategy and implications for Nursing.
- Experience of developing evidence based protocols.
- Experience of instigating investigations, interpreting investigations and initiating appropriate plan of care for patients.
- Experience of assessing patients with undifferentiated diagnosis.
- Assessment skills in history taking and clinical examination.
