This role combines social prescribing, health promotion, and smoking cessation support specifically for Gypsy, Roma and Traveller communities across Surrey.

Responsibilities

  • Build trusting relationships with GRT individuals and families
  • Provide personalised support to improve wellbeing using Make every Contact Count (MECC)
  • Deliver smoking cessation advice, behavioural support and follow-up
  • Support access to primary care, screening, vaccinations and wider services by signposting and referring into appropriate services
  • Work collaboratively with ABCs Health Inclusion Teams, VCSE partners and local authorities

Alliance for Better Care (ABC) is a GP Federation uniting 77 NHS GP member practices across 98 sites within 24 Primary Care Networks in Sussex and Surrey. We support our Primary Care colleagues — and their patients to transform how healthcare is delivered in their communities. We work closely with GP Practices, PCNs, Hospitals, Community Organisations, and the Third Sector. These vital partnerships enable us to deliver a truly integrated approach that offers the support and expertise needed to effectively serve our populations. Our Equity Team delivers clinical outreach services to people who experience barriers to healthcare, including those facing homelessness, refugees and asylum seekers, Gypsy, Roma and Traveller communities, and other groups experiencing health inequalities.
