HOURS OF DUTY: 40 hours a week

RESPONSIBLE TO: Senior Manager

MAIN CONTACTS:

  • Clients & family members
  • AICS Group staff
  • Multidisciplinary Team including: team leaders, therapists, consultants, psychologist, case managers and GPs

SPECIAL CONDITIONS OF SERVICE:

  • An enhanced Disclosure and Barring Service check will be undertaken
  • Maintain confidentiality in respect of the client
  • Able and willing to work flexibly at various locations

ABOUT THE ROLE: As a Package Coordinator, you will play an integral part as one of the key people liaising between our Clients, Support Workers and Package Managers. We are looking for someone who is confident, capable and ready for a challenge. You will need to demonstrate proven ability to coordinate people and systems and work as part of a team, supporting the Package Managers. You will be responsible for ensuring that all communication and documentation to assist our client’s rehabilitation is implemented.

MAIN DUTIES:

  • Communicate weekly with available Support Workers (SW)
  • Update the internal system with SWs availability
  • Create rotas with support worker availability and confirm availability
  • Daily/weekly/monthly/emergency shift cover and updating the internal system accordingly
  • Support Package Managers with collating necessary documentation for client visits and referrals
  • Assist and monitor with the use of I-connect
  • Liaise with support workers for MACH (Daily reports) forms / timesheets / I-connect
  • Assist Package Managers with reviews and maintain statutory paperwork (Care Plan (CP), Risk Assessment (RA), Medication Support Plan (MSP), Environmental Risk Assessment (ERA))
  • Assist Package Managers with organising and participate in staff team meetings, Multi-Disciplinary Team meetings (MDT) and Package Managers meetings
  • Assist Package Managers with creating, checking and making amendments to MACH forms using the online system
  • Regular Client contact, reviews, updates, general spot checks and to assist with Medication Competency Assessments (MCA’s)
  • Complete quarterly supervisions with SWs, reviewing personal development in line with company policy
  • Liaise with other departments to facilitate recruitment on packages
  • Regularly participate with the on-call rota
  • Send out PPE
  • Schedule Meet & Greets including finding out availability of SWs and induction of new staff
  • Schedule shadow shifts, liaising with all parties to match availability
  • Monitor documentation ensuring that all information is up to date
  • On-call duties to attend

Additional Duties:

  • Participate in the supervision and appraisal system
  • Any other additional duties required by management

SAFEGUARDING: All members of staff at The AICS Group have a safeguarding responsibility. As part of the role the post holder will undertake mandatory safeguarding as part of their induction. The post holder will be required to ensure that vulnerable adults and children are safeguarded. The role includes a duty to refer any safeguarding concerns they may have to their line manager.

TRAINING: The AICS Group is committed to offering training and support to all staff. The post holder will be required to attend mandatory training, participate in supervision, annual appraisals and development reviews.
