Poole Central Primary Care Network is recruiting an experienced Paramedic or ANP/ACP to work in our Acute Home Visiting Team on a fixed term 1 year contract. The successful candidate will provide advanced same-day clinical assessment, diagnosis and management of patients within primary care and community settings, visiting patients at home and working autonomously within a multidisciplinary team.
