We are looking for a resilient and empathetic professional to provide practical, person-centred support to households where living conditions pose risks to health, safety, and independence. Working directly with customers in their homes, you will build trust to address hoarding and self-neglect. You will manage a varied caseload, coordinating plans that reduce risk (fire, environmental, and safeguarding) while supporting individuals to remain in their homes whenever possible.

Key Responsibilities

  • Case Management: Conduct home visits and structured assessments to identify risks and implement practical interventions (decluttering, cleaning, and repairs).
  • Multi-Agency Working: Collaborate with Housing, Social Care, Fire Services, and Health partners to manage complex cases and share information appropriately.
  • Safeguarding: Recognise safeguarding thresholds, making timely referrals and escalating concerns regarding non-engagement or impaired capacity.
  • Support: Connect customers with services addressing underlying causes, such as mental health, trauma, or substance misuse.

About You

You are a reflective practitioner with a non-judgemental approach, capable of balancing client autonomy with risk reduction.

Essential Skills & Experience

  • Experience: Proven track record of working with people with complex needs (hoarding/self-neglect) and building constructive relationships with reluctant or anxious customers.
  • Risk Assessment: Skilled in using structured tools (clutter scales, hazard checklists) to assess fire and environmental risks within the home.
  • Knowledge: Strong understanding of the Care Act, Mental Capacity Act, and safeguarding pathways.
  • Resilience: Emotional resilience to handle high-risk situations, with a willingness to engage in supervision.
  • Good communication and IT skills: The ability to write clear case notes, risk assessments, safeguarding referrals and reports, and to liaise effectively with a range of partners.
