All-Age Complex and Continuing Care are a team of clinical, business, commissioning specialists who provide continuing healthcare (CHC), children and young people's continuing care (CYPCC) and complex care commissioning to individuals across the Thames Valley footprint. Our operating model has three main areas of activity:

  • Neighbourhood teams work alongside health and social care partners within the community assessing, reviewing and case managing care for adults and children within Oxford, Buckinghamshire or Berkshire, working directly with individuals and families.
  • Central services teams act as the front door and end to end service delivery, managing referrals into the service, business, administration and invoicing functions, working closely with individuals and their families.
  • Commissioning Contracting and Brokerage team work closely with care providers and social care to commission individualised care for individuals.

Main duties of the job

The post holder will provide brokerage support to the AACCC service, including CHC, CYPCC and Complex Care brokerage provision. The Brokerage Assistant plays a leading role as part of the commissioning team sourcing placements and packages of care.

Job responsibilities

  • To provide a full administration service to the AACCC Team from initial referral and throughout the brokerage process.
  • Working with a range of Care Providers to develop relationships and establish appropriate placements and packages of care to achieve the most effective patient outcomes and to ensure financial reconciliation.
  • To engage and undertake negotiations regarding fees to ensure best use of resources and value for money for the NHS.
  • To receive requests for a care service and manage the lifecycle of the placement or package of care from an initial request for a service, identifying the most appropriate Care Provider and placing with the service through standard operating procedures including mini competition processes.
  • To work with AACCC Practitioners, Case Managers and Social Workers, Local Authority, and external providers to help source, negotiate and advise of the most appropriate type of service for service users based on clinical need, outcome, cost, and efficiency.
  • To process work in accordance with internal Standard Operating Procedures and ensure that clinical and financial related documentation is entered into databases for all applicants/clients in a timely manner.
  • To liaise with families or representatives, to ensure they are kept fully informed and up to date throughout AACC processes.

Person Specification

Qualifications

  • Educated to NVQ 3 level in a relevant subject or equivalent level of qualification or significant equivalent previous proven experience.

Knowledge, Experience

  • Significant administrative/secretarial experience including initiating and maintaining office systems.
  • Demonstrable experience in dealing with the public and dealing with sensitive and confidential information.
  • Intermediate knowledge of IT systems and Microsoft applications, including Word, PowerPoint, Excel.
  • Understanding of Confidentiality and Data Protection.

  • Clear communicator with excellent writing, data entry and presentation skills and capable of constructing and delivering clear information/instructions to colleagues and/or service users.
