Patient Care Assistant (1.0 FTE, ongoing contract)

Overview

The Thames Valley Family Health Team is one of the largest family health teams in Ontario, with more than 125 physicians across 21 sites within London, and Middlesex, Oxford and Elgin counties. The team is committed to providing comprehensive inter-collaborative primary health care to over 162,000 patients.

We are seeking a skilled Patient Care Assistant to join our Integrated Primary Healthcare Team. This position will be a vital member of our team and will play a crucial role in providing comprehensive healthcare services alongside Nurse Practitioners (NPs) who serve as the most responsible provider(s) and the interdisciplinary healthcare team. The Role will involve on-site work with TVFHT’s Operations Department to offer administrative and clinical patient support in a primary care setting, with some evening hours and vacation aligned with the NP team schedule, and in accordance with TVFHT Health and Safety policies and procedures.

Responsibilities

• Assist with patient care, manage administrative tasks, and facilitate seamless communication within the team.
• Support the multidisciplinary healthcare team in a primary care setting on-site.
• Coordinate and communicate information and documentation for various meetings and events, including minute taking.
• Open and/or close the clinic as required, with some evening hours.
• Follow TVFHT policies and procedures and maintain confidentiality and PHIPA compliance.

Qualifications and Skills

• Administrative experience in a healthcare setting
• Post-secondary school diploma from a medical assistant program; Certified Medical Assistant (CMA) or equivalent preferred
• Experience providing injections and/or certificate of completion to administer injections (preferred)
• Excellent verbal and written communication skills
• Excellent interpersonal skills and experience working in a multi-disciplinary team
• Strong attention to detail and ability to prioritize work in a fast-paced environment
• Understanding of medical terminology and clinical procedures
• Proficiency with electronic medical records (EMR) systems and computer skills
• Experience with Outlook, Teams and Microsoft applications (Excel, Word, PowerPoint)
• Understanding of the Ministry of Health and Family Health Team philosophy
• Ability to work independently with minimal supervision
• Ability to coordinate and communicate information for meetings and events; minute-taking skills
• Proficiency in data gathering and compiling information for inclusion reports
• Strong critical thinking skills to anticipate and respond to administrative and clinical needs
• Commitment to protecting confidential and sensitive information with an understanding of PHIPA
• Availability to open and/or close the clinic, with some evening hours required
• Ability to monitor, track, and follow up with patients on referrals initiated by NP(s)
• Reliable transportation to travel to other locations if needed

Salary

Salary Range: $47,046 – $57,374
