The Thames Valley Family Health Team is one of the largest Family Health Teams (FHT) in Ontario. With more than 125 physicians across 21 sites within London/Middlesex, Oxford and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive, collaborative primary health care to over 169,000 patients.
The Mental Health Counsellor’s primary goals are to build the capacity of family physicians' teams to identify and treat mental health and addictions concerns as part of comprehensive primary health care. The counsellor will work collaboratively, providing team training and consultation, as well as assessment and treatment to adults, children, youth and their families. Education and skills based group facilitation is required. This role will assist the FHT in achieving its vision and mission.
Requirements- Graduate of an accredited University
- Registered and in good standing with a regulated health professional College (College of Nurses of Ontario or Ontario College of Social Workers and Social Service Workers, College of Occupational Therapists of Ontario)
- Minimum of five years clinical work experience
- Extensive mental health clinical training, generally acquired through the completion of post graduate studies.
- Knowledge and experience in mental health treatment and management (evidence-based treatments such as CBT)
- An advanced level of mental health knowledge and experience working with adults, children, youth, & families, acquired through a minimum of 3-5 years supervised clinical practice in this area.
- Understanding of Addictions care and navigation
- Excellent clinical skills and a minimum of 3 years experience assessing and managing mental health presentations commonly seen in primary care.
- Formal training and experience in evidence-based treatments such as IPT and CBT
- Experience working with various ages across the lifecycle as well as group modalities
- Responsible for teaching residents
- Knowledge of community resources an asset
- Trauma informed care
- Flexibility and a strong commitment to measurement-based care and support the “collaborative care model’ with an interest in helping build this evolving area of mental health service
- Knowledge of current mental health and privacy legislation
- Experience working in a model of care that is short term and a goal directed/measurement based care
- Group Facilitation will be part of this role
- Use of technology including virtual visits, phone, video & texting
- Work effectively, both independently and as part of an inter-disciplinary team.
- Experience in the use of EMR, Microsoft Office (Word, Excel and Outlook)
- Must possess a valid driver’s license and have access to own method of reliable transportation as travel between sites is required.

- Occupational Therapist or Registered Nurse: $34.8138 - $42.4559
