The Nurse Practitioner (MRP) will work within an integrated care model, collaborating with physicians and other interdisciplinary team members to deliver person-centred care. The Nurse Practitioner will deliver comprehensive primary care across the lifespan, focusing on health promotion, disease prevention and patient education. The Nurse Practitioner will work to their full scope of practice and serve as the Most Responsible Provider (MRP) to an individual panel of patients as well as supporting the team in the provision of same day appointments. The Nurse Practitioner will participate in the development, implementation, monitoring and evaluation of programs and quality improvement initiatives for the Four Counties Family Health Team.
Primary Responsibilities
- Provide primary health care to patients in accordance with standards set by governing body and that is aligned with the overall strategic direction of the family health team.
- Provide health promotion, disease prevention, acute episodic care and chronic disease management to patients.
- Enhance access to assessment, diagnosis and treatment within the rostered community and/or target population.
- Consult as appropriate with members of the interdisciplinary team to assess, plan, implement, evaluate and coordinate a patient’s plan of care and treatment.
- Collect, interpret and analyze data from multiple sources to formulate a medical diagnosis within scope of practice.
- Coordinate the interdisciplinary plan of care for patients by liaising with members of the interdisciplinary team to facilitate the efficient movement of the patient through the health care system; this includes leading the coordinated care planning process.
- Provide leadership in the development, implementation and evaluation of quality management programs.
- Participate and provide leadership on committees.
- Contribute to the development/implementation and evaluation of patient education programs.
- Conduct educational teaching sessions with individuals, groups of patients and/or families with a focus on health promotion, disease prevention and impact of illness experience on patient and family.
Qualifications
- Current Registration with the College of Nurses of Ontario in the Extended Class.
- Membership with the Registered Nurses Association of Ontario.
- Completion of a Master of Nursing– Primary Care Nurse Practitioner Program (PHC-NP certificate).
- Minimum 2 years of most responsible provider NP experience in primary care or community health.
- Strong assessment, clinical judgment, and communication skills.
- Ability to work independently and collaboratively in a multi-disciplinary team.
- Computer literacy, including use of Electronic Medical Records (EMR) - experience with TELUS PS Suites an asset.
- Excellent communication, leadership, critical thinking, organizational skills.
- Experience in program development, implementation, monitoring and evaluation.
- Ability to work extended hour hours, including after hours clinic.
Hourly Rate: $56.63 - $69.06 (based on years of experience).
