As part of the Primary Care Action Team (PCAT) initiative to attach Ontario residents to primary care, TVFHT is seeking a physician consultant to support expansion work through one of our Interdisciplinary Primary Care Team (IPCT) locations. The Consulting Family Physician will provide expert consultation and collaborative support to primary care Nurse Practitioners (NPs) serving unattached patients and patients attached to NPs as most responsible provider. The Consulting Family Physician will work closely with the primary care NP and team to ensure comprehensive and optimal primary healthcare services, leveraging a stepped approach to consultation. The consulting physician’s expertise and guidance will be integral in supporting the NP-led model of care delivery, fostering a collaborative environment, and maintaining standards of excellence.
Responsibilities
- Provide timely and expert consultation to NPs when care is outside NP scope of practice, complex patient cases, diagnostic challenges, and treatment plans within stepped consultation approach.
- Review patient records, diagnostic tests, and treatment plans to provide comprehensive recommendations.
- Authorize medical directives for NP implementation if required.
- Provide clinical opinions and support for patients with complex medical needs across a diverse range of health conditions and ages.
- Serve as a resource for clinical education and mentorship to NPs and team, promoting professional growth and development.
- Ensure that patients receive high-quality, evidence-based care through collaborative consultation and delegation.
- Support the clinic’s mission to provide primary healthcare services to unattached and vulnerable populations in London Middlesex.
- Order or sign off on procedures or certain forms/tests that are outside the NP’s scope.
- Advocate for patients' healthcare needs and support NPs and team to coordinate with community resources to enhance patient care.
Quality Improvement
- Participate in quality improvement initiatives aimed at enhancing patient care, clinic efficiency, team function, and healthcare outcomes.
- Contribute to the development and implementation of clinical workflow to provide efficient consultation.
- Monitor and evaluate the effectiveness of patient care protocols and provide feedback for continuous improvement.
Qualifications
- Medical Degree (MD) from an accredited medical school.
- Current licensure with the College of Physicians and Surgeons of Ontario (CPSO) that is in good standing.
- Be an Active Member in good standing with the College of Family Physicians of Canada (CFPC).
- Minimum of 5 years of clinical experience in a primary care setting preferred.
- Experience working with vulnerable and marginalized populations, including management of addictions is preferred.
- Experience working in a multi-disciplinary setting is preferred.
- Strong clinical and diagnostic skills with the ability to provide expert consultation.
- Excellent communication, interpersonal, and teamwork skills.
- Commitment to continuous professional development and evidence-based practice.

- Strong organizational, time management, and problem-solving skills.
- Ability to work independently and collaboratively within an interdisciplinary team.

The IPCT Consulting Family Physician will work virtually supporting London IPCT Stand-Alone Clinic Nurse Practitioners and teams. May require occasional travel for meetings, training, or community outreach events. The clinic provides a supportive and collaborative environment with a focus on delivering high-quality care to unattached patients and NP MRP attached patients.
RATE: $1800/month (Anticipate 2 hours/week time commitment)
