About us: Thames Reach runs projects and services that help to end homelessness in and around London. We work directly with homeless people on the streets and in hostels, prevent vulnerable people from becoming homeless, help formerly homeless people get their lives back on track, and help people find work and access training and volunteering opportunities.

About this role: We are looking for a Lead Worker to join the Resettlement Support Team. Thames Reach’s hostels are transitional spaces where a 24‑hour staff team supports people to move in, assess their needs, and quickly identify the best place for them to move on to next. Skilled staff develop personalised support plans to ensure people are ready to move on and are accessing support with their health, substance misuse, criminal justice and life skills needs.

You will:

  • Take the lead on assessing clients and devising and implementing sustainable move‑on plans to support people into more independent/appropriate accommodation.
  • Take the lead on building and sustaining positive professional relationships with participating partners in the pilot.
  • Link clients into post‑resettlement support to help ensure tenancies are sustained.
  • Take the lead and represent the team at multi‑agency meetings.
  • Accurately record all work completed within given timeframes.
  • Take the lead within the team to contribute to data collection and reporting.

You will have:

  • A strong commitment to providing a high standard of service to both clients, funders and partners.
  • Experience working with vulnerable adults and supporting them to access and maintain engagement with services.
  • Excellent organisation and time‑management skills to remain outcome‑focused, with the ability to coordinate a high number of cases and complete tasks promptly and efficiently.
  • Experience in forming strong working relationships with partner agencies, being able to challenge them when required.
  • Strong IT skills with the ability to use Outlook, online recording systems, Excel and Word, using a tablet and a smart phone.
